This paper discusses tools which can identify key organisational factors which contribute to aviation accidents.  The research case study is a thoroughly-investigated helicopter accident, used to determine the extent to which analytical and visualisation tools could be used to assess maintenance organisational issues which contributed to prevention of an accident.
