Analysis of accidents indicates that the advances in safety developed in the late 1990/2000’s have been exhausted and that new ideas are needed. “Safety Management Systems” are the latest development worldwide. This paper analyses recent accidents to develop ideas for accident prevention through more effective Safety Management Systems.
