A multi-fatality accident involving Singapore Airlines Flight 006 (SQ006) is analyzed with the Rasmussen-Svcdung ACCIMAP tool to represent the players, active and latent conditions in the system that interact to shape the error causal topology and to understand the forces that influences airport safety management.
